WITH GUIDE, PEOPLE WITH DEMENTIA AND THEIR CAREGIVERS CAN ACCESS MORE HELP AND SERVICES

  • A full health check-up from a Nurse Practitioner
  • A personalized care plan designed for people with dementia
  • One-on-one and group education for both caregivers and beneficiaries
  • 24/7 phone access to a nurse or Nurse Practitioner for questions or concerns
  • Help with:
    • Scheduling doctor’s appointments
    • Advanced Care Planning
    • Managing medications
    • Getting specialist referrals
    • Coordinating post-hospital care
    • Accessing services like meal delivery, transportation, and more
    • Respite care (if eligible) — short-term help to give caregivers a break

FREQUENTLY ASKED QUESTIONS

What is the goal for GUIDE?

GUIDE focuses on enhancing the quality of care, supporting caregivers, and improving communication between healthcare providers, patients, and families. The program supports the ability of individuals with dementia to remain safely and comfortably in their homes and communities for as long as possible. 

Who can join GUIDE?

People who are enrolled in Original Medicare and have dementia, Alzheimer’s Disease, or another dementia-related diagnosis. A caregiver is not required to be eligible for GUIDE; if you are managing your own health we can help.

Who is considered a caregiver?

A caregiver can be a family member or an unpaid person (like a neighbor or friend) who helps someone with dementia. This help can include things like bathing, dressing, meals, or managing appointments. You do not need to live with the person or help them every day — even occasional support may qualify.

Does this cost anything?

No. This is a free benefit for people with Original Medicare.

Can the person still see their current doctors?

Yes. They can keep seeing any provider who accepts Medicare. We’ll also work with their care team to keep everyone informed.

What if the person already receives home care through Medicaid or pays for it themselves?

That stays the same. We’ll coordinate with existing home care aides, agencies, and Medicaid Care Managers. Respite care may also be available to help when extra support is needed.

What if the person has a Medicare Advantage plan?

The GUIDE program is only for people with Original Medicare. But Vesta offers other helpful, no-cost programs that they may qualify for. Call us to learn more.
